Creating and Configuring Surveys

Create, configure, and share standalone forms to collect data and manage feedback independently of your program workflows.

The Surveys Module is a tool for creating standalone external forms. Unlike Program Submission forms, these are independent of specific application workflows and are used to collect data or feedback from participants, judges, or external stakeholders.

I. Accessing the Surveys Module

  1. Log in to your Dashboard and navigate to Workspace.

  2. Select the relevant Platform and click Manage.

  3. In the sidebar, click on Surveys.

  4. Click the + New Form button.

II. Initial Setup

When creating a new survey, you must define the following:

  • Form Name: The title of the survey as it appears to respondents and in your dashboard.

  • Slug: The unique part of the URL used to share the form.

  • Description: A short explanation of the survey's purpose.

II. Configuring Survey Settings

Once the form is created, you will be directed to the configuration area, which is organized into four main tabs.

1. Settings

This tab controls the basic parameters of the form:

  • Form Title & Slug: Update the name or URL at any point.

  • Description: Edit the instructions or introductory text.

  • Start Date and Deadline (optional): Define the window during which the survey will accept responses or leave it empty to always accept submissions.

  • Multiple Submissions: Allow or prevent users from submitting the form more than once.

  • Select Program: Link the survey to a specific program within your platform.

  • Set Visibility: Manage access permissions for who can view and fill out the form.

2. Messages

This tab manages the confirmation process after a user submits the form:

  • Submission Confirmation: Customize the text displayed to the user sent immediately after submission.

  • Auto-Send/Attach Copy: Configure the system to automatically email a copy of the completed submission to the respondent.

3. Form Builder

This tab contains the drag-and-drop interface used to design your survey questions. You can add various field types such as short/long text, multiple-choice, rating scales, and file uploads to structure your data collection.

4. Submissions

This is the data management hub where you can view all individual responses in real-time. This tab allows you to monitor entry counts, review specific answers, and export the complete dataset for external analysis.
